KEYWORDS: Optiview, Optical access trocar,
pneumoperitoneum, Veress needle, incisional hernia. NO. OF
PATIENTS: 650. OBJECTIVE: To determine whether the Bladeless Trocar can
be used to effect safe and rapid entry in various laparoscopic procedures. KEY
STATEMENTS: The Bladeless Trocar
provides the basis for safe and effective initial trocar placement compared to
blind entry. The authors experienced a 0.3% rate of
complication in 650 patients compared to rates of complications due to Veress
of from 1% to 8.4%. The optical
trocar provides for visualization, and lack of vision is a common denominator
in inadvertent injuries during initial entry in laparoscopic access. Since tissue plans are transected by
dilatation rather than cutting, the risk of incisional hernia is smaller.
